To activate your kit, please go to https://app.forthwithlife.co.uk/activate and type in your activation code. You can find your activation code in the top right-hand corner of your lab request form.
If you have sent your sample before activating your kit, all is not lost. Get in touch with us by emailing help@forthsupport.co.uk, and our customer support team will assist you in activating your kit. This will then allow you access to your secure Health Dashboard to view your results.
